Our Professional Guarantee
All physiotherapy treatment will be given only by chartered physiotherapists who are recognised members of the CSP (Chartered Society of Physiotherapy) and members of the HPC (Health Professions Council). In addition they may also belong to the following associations: PhysioFirst (the Organisation of Chartered Physiotherapists in Private Practice); ACPSM (the Association of Chartered Physiotherapists in Sports Medicine); AACP (the Acupuncture Association of Chartered Physiotherapists); SOM (Society of Orthopaedic Medicine).

All other practitioners working at PhysioActive are fully qualified and members of their respective professional bodies.

Please note that we require your consent before we give you treatment. Your signature on the registration form confirms that you have read and understood our terms and conditions. It also confirms that you agree to our payment terms.

Payment Terms
We appreciate prompt payment. All treatment should be paid for on the day it is received unless your account is guaranteed in advance by an agreed third party.

• How to pay: Payment can be made by cash, cheque, credit/debit card.

• Insured Patients: PhysioActive is recognised by all UK medical insurance companies and we are happy to bill your insurance company directly for treatment you receive from us. In order for us to do this, please contact your medical insurance company directly and provide us with your policy details and an authorisation number before your first appointment. We regret that without this information we will require payment from you directly and will provide you with a receipt to claim back the costs from your insurance company yourself.

If we are billing your insurance company on your behalf, we will require your credit/debit card details when you register with us. This allows us to bill your card directly should your insurance company not settle the bill in full, or in case of an excess on your policy. We will write to you before we charge your card and provide you with a receipt once the bill has been paid. Please be assured that all credit/debit card details are kept securely and confidentially at all times and will be destroyed from our system at the end of treatment and once your bill has been settled in full.

If you would prefer not to give us your insurance details, we will require payment from you at the time of treatment and will give you an invoice so that you can claim back the costs directly from your insurance company yourself.

Please note that it is your responsibility to keep track of your account and ensure that you are covered by your private insurance for all treatment received. For example, you may be allowed only a limited number of treatment sessions, or there may be a maximum amount your insurer will pay. Also, few insurance companies cover acupuncture and podiatry, so please check first.

You are ultimately responsible for the cost of your treatment. Should any insurance claim be declined or only partly paid by your insurance company, we will invoice you for immediate payment of the outstanding balance.

• Payment by a third party: PhysioActive will accept direct settlement from a third party, such as your employer or solicitor, providing we have an arrangement with them in writing prior to treatment taking place.

Cancellations and missed appointments
PhysioActive operate an industry standard 24 hour cancellation policy. Like you our time is valuable and so we ask you to give us at least 24 hours notice if you cancel an appointment. All missed appointments and appointments cancelled within 24 hours of the due appointment will be charged a £25 cancellation fee. Please note that insurance companies do not usually pay for these charges and you will be responsible for paying the cancellation fee directly.

Patient Confidentiality & Data Protection
PhysioActive Ltd is guided by the Data Protection Act 1998 which sets rules for the processing of personal information. You have the right to access information held about you either on computer or in manual records. If you would like this information, please contact us and we will send you a ‘Release of Medical Records’ form to sign and return. There will be a small administration fee for this.

PhysioActive Ltd is committed to maintaining strict patient confidentiality. This means that when accessing patient information we do the following:-
• Justify the purpose
• Use patient identifiable information only if it is absolutely necessary and legally permissible.
• Ensure that patient identifiable information is accessed only on a strict need to know basis.
• Ensure that everyone with access to patient identifiable information is aware of their
responsibilities.
• Understand and comply with the Data Protection Act 1998
